MEDICARE:
We accept patients on the Medicare plan. As a courtesy, we will also bill your secondary insurance provider directly. You will be asked to sign an Advanced Beneficiary Notice for services not covered
under Medicare. You shall be accountable for covering these services.
OUT-OF-POCKET AND UNINSURED PATIENTS:
We provide basic primary care for you and your family, at discounted rates when paid at time of service for those who are cash-pay patients. We provide care to anyone in need of
primary care services within the areas we cover, and at the various locations we designate.
Our discount rates vary according to the the level of medical complexity of care:
Level 1: $25
Level 2: $35
Level 3: $55
Level 4: $80
Level 5: $95
Other Insurances: Bridgeway and SCAN insurance plans pending to better serve our group home patients.
Our services in focus
Check-ups
Routine check-ups are more important today than ever before. Your group home residents should be seen regularly to be sure that their medications are reviewed and health status is evaluated. But don't forget about yourself! Regular health examinations for you will hopefully catch a condition early when it is most manageable.
Ultrasound tests
Also known in medical jargon as sonography, ultrasonic testing provides early information about the condition of your inner organs.
We also offer the following services and have the network to order and coordinate the following services:
- Primary Care
- Prescriptions and Medication Management
- Lab tests: ordering and interpreting
- XRays, EKG, and Ultrasounds
- IV Therapy
- Arrangement for annual vaccinations
- Referrals to specialists
- Tube Feeding
- Home Health
- Hospice care
- Non-urgent medical transportation
- In-home non-medical assistance such as house chores, assistance with meal preparation, grocery shopping, etc.
