As a collaborative and networking practice, we offer a broad range of services in primary care to group home residents, group home caregivers, residents of assisted living facilities, and to uninsured patients. About Us
Triad Clinic uses a Case-Management model in delivering patient care. If our patient needs a referral to a specialist, we will facilitate that. If our patient needs admission to a hospital, we will facilitate that process by keeping our patient within our network of inpatient physicians. We also assist with transportation arrangements. Our case management will follow our patients throughout the healthcare system, to provided continuity of care and assist in effective communication between members of the healthcare team, and appropriate use of resources and referrals.
